About The Role
Are you an Occupational Therapist, Physiotherapist, Nurse or Social Worker experienced in working with disabled children, young people and their families based in the Bristol area? Have you considered a career in Case Management?
 
The role is offered home-based with consideration to full or part-time hours. You would manage a client caseload with visits to clients in their homes within your geographical area. 
 
Working remotely for ILS doesn't mean working alone. Our Case Managers are supported by a dedicated Professional Mentor who is experienced in case management. You will receive regular mentorship sessions and the assistance of a knowledgeable Head Office team.

About You

We require health and social care professionals who have extensive expertise of working with children and young people with catastrophic injuries; including traumatic brain injury, poly-trauma, spinal cord injury, limb loss and cerebral palsy.

You will have experience of working directly with clients in the community and be able to demonstrate your professional abilities, within the field of complex, long-term health care assessment, care planning and achieving  positive outcomes for clients.

You will have current HCPC/NMC/Social Work England registration and an up to date CPD portfolio.
