You will be a health and social care professional (Occupational Therapist, Physiotherapist, Social Worker or Nurse) with extensive expertise of working with adults with complex needs in challenging situations.
You will have experience of working directly with clients in the community and be able to demonstrate your professional abilities, within the field of complex, long-term health care assessment, care planning and achieving positive outcomes for clients.
You must have current HCPC/NMC/Social Work England registration and an up to date CPD portfolio.
