Job Summary
Hi, I’m Tristan!
I was born in 2000 and live with complex physical, psychological, and communication needs due to an injury at birth. But that’s just one part of my story - I’m fun-loving, sociable, and I love getting outdoors, shopping, attending day services, and having a good laugh with my support team. Keeping things upbeat, engaging and active are really important to me. We have a dog, that likes to bark but is very friendly.
I live in an adapted annexe off of my parent's house, it is a rural area so being a car driver is essential. I use a wheelchair and am gastrostomy fed, so confidence in these areas is essential. I rely on my carers and parents for all of my daily needs.
Main Duties and Responsibilities
- Supporting young adults with neuro disabilities
- To provide safe, effective care and support to me in my home and out in the community
- To ensure that care given by the team is of high quality, safe and effective
- Calm, proactive and can bring a sense of fun to my day
- To follow a 24-hour posture management programme
- To manage my medication and feeding regime
- To ensure that my equipment/immediate home environment is kept tidy, organised, and in good order
