About The Role

We are seeking case managers to compliment our team in Kent.


Based from home, you would manage your client caseload including travel and client visits within your geographical area and within the hours that suit you and the client’s needs.


The role is offered on a part-time basis and could be employed or self-employed depending on the individual circumstance.


All case managers are supported by our Head Office team who provide administration, IT and technical support. We have a team of qualified and experienced HR and payroll professionals to support the case management process.


On joining, you will receive acomprehensive induction and have regular, mentorship, ongoing training andprofessional development opportunities.

About You

We require Health and Social Care HCPC or NMC registered professionals who have extensive experience of working with children and adults with catastrophic injuries; such as traumatic brain injury, poly-trauma, spinal cord injury, limb loss and cerebral palsy.

You will have experience of working directly with clients in the community and be able to demonstrate your professional abilities, within the field of complex, long-term health care assessment, care planning and outcomes for clients.
