About The Role

Location: Farnham, Surrey
Days & Hours: RGN Co-ordinator: 
Thursday Waking Night shift every week, 8pm - 8am, One Sunday day shift per month, 7am - 7pm (may start at 8am depending on cover) with a minimum of 3 hours per week for Nurse Co-ordinator administrative duties. 

Waking Night Support Worker: Part time Permanent shifts available
Week 1 - Wednesday, Waking Night, 8pm – 8am and Saturday, Waking Night, 7.30pm - 8am
Week 2 - Monday, Waking Night, 7:30pm to 7:30am

Please note that additional shifts on the 24 hour rota may be available to cover annual leave and sickness of the current Support Team.

Pay: Nurse Co-ordinator: £31.00 per hour, Monday - Sunday  Waking Night Support Worker: £14 per hour, Monday - Friday and £16 per hour, Weekends

Melanie, born in 2008 lives with her Mother, Father and younger brother in their adapted home and is currently being educated at home. She really enjoys carrying out fun activities such as music, cooking and art. As Melanie is visually impaired, she is predominantly drawn to sensory activities. She loves music, dark rooms with bright and sparkly lights, especially blue ones which she is able to focus on. Melanie also likes familiar routines for activities and experiencing the world through her senses. She likes team members who she knows and who know her well.
 
Melanie is non-verbal and will use switches to obtain attention. This is only successful if she is not tired and she is in the mood to engage. Melanie uses her body language, facial expressions and some vocalised sounds to make her needs known.
 
Melanie is looking for a *Female Nurse Co-ordinator and a *Female Waking Night Support Worker to help with all aspects of her daily life.  She has profound health needs and has a diagnosis of Spastic Quadriplegic Cerebral Palsy, Epilepsy, Scoliosis, Lymphoedema (fluid build-up & swelling), severe (Cortical) Visual Impairment, severe Learning & Communication difficulties and Osteoporosis. Melanie has a Gastrostomy, Tracheostomy and is jejunely fed on continuous pump feed. Melanie is completely reliant on others for all of her needs and for the planning of her day so that she can reach her full potential and to be as happy as possible. 
 
Nurse Co-ordinator
We are seeking a patient and caring part-time Band 7 (or above) *Female Nurse Co-ordinator to support Melanie during the day and at night. Your role as Nurse Co-ordinator will be to lead and support Melanie's care team including the organisation of rotas, liaising with her family and many health professionals involved in her care, providing supervision and support for the team, as well as being involved in the recruitment of new team members.   You will also provide hands on care supporting Melanie throughout the night and during the day. You’ll be required to monitor her, administer medication and feeds and provide all aspects of personal care (including ensuring her tracheostomy management and care is at the highest level). Your role during the night will involve repositioning Melanie and to reassure and comfort her, due to her having disturbed nights where she often wakes during the night. You will be required to monitor her throughout the night in conjunction with another Support Worker and keep accurate notes. The role will involve all aspects of personal care including the morning routine until the day Support Worker arrives, this will include giving medication, liquid and pump feeding, changing incontinent pads and bed linen. 

Waking Night Support Worker role
The Waking Night Support Worker will assist the other Support Worker on shift with repositioning Melanie when needed, cleaning down equipment, making medication, assisting with liquid and pump feeding, changing incontinent pads and domestic chores. The Waking Night Support Worker will be expected to work co-operatively and implement support as advised by Melanie's Mother and Father, the Nurse Co-ordinator and her team of therapists and record information relating to care and attention given.

Must be able to work with specialist equipment (training will be provided). A Handling Management Plan is
available and is designed to reduce the risk of handling injuries, but you may be required to bend, kneel, squat, push or pull.
Confident about working in the Melanie's own home and sensitive to the needs and wishes of the family.
Experience of working as a Support Worker with clients with additional care needs.

Full driving licence (endorsements to be disclosed) and able to drive the client’s adapted vehicle.  
Good written skills and the ability to keep adequate written records of care given. 
Punctuality is essential to ensure smooth running of the care regime. Any unforeseen circumstances which would cause delay should be reported to the people on duty as soon as possible.
Comfortable to be around dogs, (as our client has one).
Non-smoker - this post operates a non-smoking environment.

RGN Co-ordinator (including Support Worker skills/behaviours above) 

Relevant extensive nursing experience: Band 7 minimum. 
  •      NMC registration and demonstration of post registration experience.
  • Evidence of Continuing professional Development.
Registered Nurse with experience in the tracheostomy care and management and epilepsy, preferably also experienced in working with children.
To be competent in Gastrostomy-jejunostomy Peg Feeding 
Experience of working in a supervisory role. 
Experience of formulating appropriate care plans and risk assessments for clients.
Able to maintain effective working relationships, including the utilisation of the appropriate person management skills required to lead, train and support a team of support workers.
Good communicator: Communicates very sensitive, complex condition related information to Melanie, family, empathy, reassurance required. 
 

Satisfactory references & proof of eligibility to work in the UK required.
